If you are renting facilities from the Canadian Baptist of Atlantic Canada (CBAC) member church and you need to provide proof of Commercial General Liability (CGL) insurance, use this form to buy your insurance. A Certificate of Insurance is issued to you as the Named Insured, providing assurance to the Certificate Holder (CBAC), and their related companies (Additional Insured) that a CGL policy is in place to cover your event.
